Addiction treatments that are inpatient are more structured as the addict resides at the facility.  Inpatient programs will have daily schedules as well as lots of structure in an attempt to distract the addict from the obsession of drugs and alcohol.  Inpatient facilities can either have a very medical/clinical approach or a more laid back approach.  Either can be effective depending on the individual case.
